This clinic now supportsonline insurance verification(using your My Number card as your health insurance card), from 8 October.
Kiyose Asahigaoka Danchi Clinic has put arrangements in place for online insurance verification, and can now obtain and use patients' visit history, medication information, specified health check results and other necessary information when providing care.
As well as the usual health insurance card, a My Number card can be used as your health insurance card once you have completed the first-time registration.
You can still be seen with the usual health insurance card. For details, please seethe Ministry of Health, Labour and Welfare page on using a My Number card as a health insurance card.
Once a month we check your insurance details. Please hand your health insurance card or My Number card to the reception desk.
For online insurance verification, your personal information may also be used to make enquiries to the review and payment body or to your insurer.

This clinic obtains and uses the information needed for its medical DX arrangements and for providing high-quality care.
We submit claims online and have arrangements in place for online insurance verification.
Through the online insurance verification system, our doctors and staff can obtain and use patients' medical information.
